Involve Kent
Involve Kent is a charity that aims to improve health by empowering people and communities to live well and tackle health inequalities such as; frailty, disadvantage, loneliness and isolation. The services that Involve Kent provide are based on the needs of the community. For example; Actively Involved helps people over the age of 55 (or younger if they have a long-term health condition) to connect to activities in their local area, such as; theatre trips, coffee mornings, lunch clubs etc to help people live a happy, healthy and connected life. Community Projects, is a division that responds directly to demand by creating groups and sessions that would benefit the community. Examples of these projects are; free postural stability classes to help build muscle, help mobility and balance, supported volunteer programmes to help people build confidence, learn new transferable skills and gain experience. Children\’s Health & Wellbeing Navigation Service is an essential service that works with a variety of GPs across Kent to provide Children\’s Health & Wellbeing Navigators that can help children up to the age of 18 (or up to the age of 24 if they are on the Autistic Spectrum Disorder Spectrum) by connecting them to resources and activities within their local areas. Involve Kent are paving the way with their innovative Social Prescribing Service that aims to provide regular one-to-one supportive sessions with patients that have been referred by GPs. The support can include mental health support, housing support, transport, exercise classes, gardening groups, singing groups and much more. Involve Kent also provide guidance and support to carers within Kent, which includes; signposting them to resources, mobilising them by informing them of their rights, assisting with hospital discharge, Carer Card applications, mobility and accessibility assessments and much more. Involve Kent take a holistic view of someone\’s health, meaning they look at what matters to you, how your health is affecting your quality of life and what other factors may be affecting your health and wellbeing.
